§ 93.03. LICENSE APPLICATION.

Written application shall be made in writing at the office of the City Clerk/Treasurer. The application shall state the name and address of the owner of the dog, the name, breed, age, sex, and color of the dog, and be accompanied by a certificate of vaccination from a qualified source certifying that the dog has been vaccinated against rabies, or has received a booster shot therefore, with the 24 month period immediately preceding the application.
